Please complete the student information form, medical form, and free/reduced lunch form and return them in the pre-paid envelope. You may also drop the forms off at your students’ building.

Congratulations to our Mainline School Based Health Clinic for winning the highest quality care award for school clinics! We're so grateful for the services they provide our district! Our middle school mainline location will be open beginning Monday, 8/17 for back to school physicals and immunizations.
